How to Register

​

To register with our practice, please download, print and complete the relevant form below:

​

• Adult Registration form (aged 16 years and over only)

Once you have completed the form you will also be required to complete a GMS1 registration form, we also require two forms of ID.  You can attend the practice to drop all of these documents off or you may also send all of the documents via email to

lscicb-gp..drralisurgery@nhs.net.

Alternatively, you can visit reception and complete the registration forms in person; please bring your ID with you.

 

What identification (ID) do we need?

Acceptable Identification so that we can check the details on your form are accurate. We need two forms of ID, one photo and one proof of address. Examples of these include:

• Tenancy agreement

• Mortgage agreement

• Bank statement

• Driving licence, passport, birth certificate etc.

Until you have submitted your GMS1 form and had your ID checked you will not be registered at the practice.

What happens next? 

​

Once you have been registered, you will receive a text message from the surgery to confirm this and from here, you will be able to request medications and book appointments at the Surgery. 

​

Registrations are completed once a week by a member of staff. Therefore, please allow up  to 7 days for your registration to be completed.
